Why are some regimes so brutal to their people?

why are some regimes so brutal to their people? such as North Korea and Khmer Rouge. North Korean people are so poor and are in the brink of starvation, but the regime seems to not care about the people's desperation. why are there regimes in the world not caring about their people's welfare even though their people are living in desperate conditions?
